About the PHQ-9

The Patient Health Questionnaire-9 (PHQ-9) is a validated 9-item screening tool developed by Spitzer, Kroenke, and Williams (1999) and used by clinicians worldwide to screen for and monitor the severity of depression. It is based on the DSM diagnostic criteria for major depressive disorder.

This tool is for educational screening purposes only. A PHQ-9 score alone cannot diagnose depression — that requires a full clinical assessment by a licensed provider. If you score in the moderate or higher range, we strongly encourage you to speak with a mental health professional.
